Job Description:
A company is looking for a Transition Specialist to manage the
coordination of member transitions in the community.
Key Responsibilities
Support care coordination teams to develop effective transition
plans for members
Assist with member transitions based on enrollment or care service
changes
Track transition metrics and ensure proper documentation of
transition care information
Required Qualifications
Bachelor's degree required
2 - 4 years of related experience preferred
Equivalent experience may be considered in lieu of a degree
Knowledge of care coordination and community health services
Experience in developing educational materials and training
programs
