Description:Pay Rate: $40 - $75 per visitJob Summary: Licensed Vocational Nurse (LVN)American Hospice and Home Health Services is currently seeking a Full Time, Part Time, and/or Per Diem LVN to service Napa County.*Negotiable to salary or hourly pay rates depending on experience.Our home health care mission is to provide professional and paraprofessional services to patients in their homes, assisting them to achieve the highest level of potential in their day-to-day activitiesEssential Job Functions/Responsibilities:As the Hospice and Home Health LVN, you will provide nursing care and supportive services in accordance with the physician/provider treatment plan and the nursing process of assessment, diagnosis, planning, implementation and evaluation under the supervision of the Registered Nurse. Performs accurate assessments, interprets and correlates findings, establishes plan of care, observes patient progress, records pertinent observation, implements physician and nursing treatment plans and evaluates outcomes.Responsibilities and DutiesAbility to complete all documentation in accordance with policy and procedure.Flexible, as varied daily activities will include phone, email and written communication, meetings with a variety of healthcare professionals, great understanding of hospice rules and regulationsMust work closely with team members to provide the optimal care and services.Ability to manage scheduling visits, documentation time, and meetings effectively.Ability to learn an electronic medical records system.Requirements:Qualifications and SkillsLicensed Vocational Nurse License in CaliforniaPreferred 1-2 years Professional Experience in Home Health and HospiceCurrent CPR CertificationHome Health experience preferredIV CertificationWound Care and/or wound vac experience preferredAbility to drive within 30-60 miles a dayAbility to pass drug and background screeningsAbility to function autonomouslyStrong clinical skillsEffective communication skillsProficient computer skillsReliable transportation and insuranceCOVID-19 Precaution(s):Temperature screeningsSanitizing, disinfecting, or cleaning procedures in placeCOVID-19 Vaccination required per agency policyEnvironment:Home HealthCommunity based servicesHospice at HomeWork RemotelyNoThis Job Is Ideal for Someone Who Is:Dependable -- more reliable than spontaneousPeople-oriented -- enjoys interacting with people and working on group projectsAdaptable/flexible -- enjoys doing work that requires frequent shifts in directionCompensation details: 40-55 Hourly WagePIeb377b340662-25660-36572367
