Job Description:
A company is looking for a Chronic Care Management Registered
Nurse (Remote).
Key Responsibilities
Plan and conduct intervention opportunity evaluations and respond
to alerts to ensure high-quality care
Develop and update care plans while monitoring caregiver and family
support
Collaborate with internal teams to promote quality outcomes and
optimize service experience
Required Qualifications and Education
Current RN license in New York AND Compact
Graduated from an accredited nursing program
At least 2 years of nursing experience with adult and geriatric
populations
Confidence with clinical skills and knowledge of chronic
conditions
Ability to work remotely with a private workspace
